 * Orders require prepayment, except in the case of U.S. government 
   agencies, recognized U.S. Public libraries, universities or colleges.

 * Telephone and FAX orders may be made using VISA or MASTERCARD.  
   Include number and expiration date of card.

 * Indicate "Catalog Book Number" (e.g. C-2, C-17, etc.) for each book.
   Some books are available in both soft cover and library bound.  (Note
   that library bound books are softcover books with sewn pages enclosed
   in heavy-duty laminated covers.)  Check the appropriate box for type
   of binding ordered.

 * Shipping/handling charges in U.S./Canada:
                          First book $5.00; each additional book $1.00

 * For shipments outside U.S. and Canada:
                          Surface shipping/handling $7.00 per book
                          Air shipping/handling $16.00 per book

 * Remittances must be in U.S. dollars with payment against a U.S. bank.

 * California resident add 8-1/4% sales tax.
